About COHYA

Center of Hope Youth Advocacy (COHYA) was established in 2009. We believe that families should have guidance and support to assist with life challenges. Our mission is to prevent destruction in the family unit and help children to become successful, self-sufficient adults through counseling, case management, and mentoring services. 

Service Areas

COHYA provides Targeted Case Management (TCM) in the following areas of concentration:

At-Risk Children

Children impacted by offending or victimization.  A victim of abuse, neglect or violence. Medical complexity that requires frequent care planning. (Learn More)

Medicaid eligible pregnant women who are at-risk for medical compromise (Learn More)

Psychoactive Substance Disorders 

Medicaid eligible individuals who are at-risk of substance abuse, dependency or addiction or
diagnosed with a substance disorder, psychoactive substance dependency (Learn More)

Intellectual and Related Disabilities

South Carolina Medicaid eligible individuals who have a suspected diagnosis of intellectual

Substance Abuse

Families with a lack of mutual attachment and nurturing by parents/caregivers with a family history of alcoholism. A chaotic home environment with substance use in-home. (Learn More)

At-Risk for Genetic Disorders 

South Carolina Medicaid eligible individuals who have been diagnosed with a genetic disorder, pr showing evidence of a disorder and individuals who have a family member with an illness that is associated with a genetic disorder. (Learn More)
